As a Child Care Provider,
How do I enroll?
In Arizona, you may participate in CCFP in
one of two ways. If you are a child care home provider,
relative child caregiver, respite care, or DES/DHS provider,
you are eligible to participate. A child care provider who
is not state-certified can participate in the food program
through Alternate Approval (AA), as long as you meet the
following requirements:

A health and fire inspection of your home must be completed.
Our office will make all the necessary arrangements.
A Department of Public Safety Class 1 or
Class 2 fingerprint clearance card must be obtained by the
provider.
Or
If you are State-certified, a copy of your
certificate from either the Arizons Department of Economic
Security (DES) or Department of Health Services (DHS) must
be submitted with your CCFP enrollment application.
How does the reimbursement part of CCFP
work?

reimbursement rates, effective July 2002 until June 30, 2003
